What you will do
- Conduct comprehensive biopsychosocial assessments and develop tailored care plans to address medical, behavioral, and social needs.
- Navigate complex federal and state programs including SSI/SSDI disability, CalFresh, and IHSS to secure essential resources for your patients.
- Manage a dedicated panel of over 80 patients, providing ongoing education and monitoring to maximize patient satisfaction and care efficacy.
Who this is a fit for
- Holds a Master’s degree in Social Work, MFT/Counseling, Psychology, or a related field, which is required for this level of specialized care management.
- Brings at least one year of experience in outpatient behavioral health or social support, with a preference for two years in direct care management.
- Possesses deep knowledge of California’s Medicaid landscape and federal benefit programs, with the ability to manage a large patient panel effectively.
